ICWP MANAGED SERVICES

Ivy Health Case Management is dedicated to ensuring individuals receive the right services at the right time. Through comprehensive oversight, coordination, and monitoring of waiver-approved supports, we help safeguard each member's health, safety, welfare, and personal goals while promoting successful community living. We coordinate and manage the following services:

Temporary relief for primary caregivers through professional support for individuals who need help with daily living. It allows families to rest and recharge while ensuring their loved ones receive safe, compassionate, and high-quality care

Treatment and healthcare procedures ordered by a Physician to provide care within the scope of nursing such as dietary practices, sanitation, and use of emergency medical services

Specialized Medical Supplies include medically necessary items and equipment  may include incontinence products, nutritional supplements, mobility aids, adaptive equipment, and other physician-recommended supplies directly elated to the member’s assessed medical needs

Services that provided physical adaptations to homes (non-rental) that are necessary to ensure the health, welfare and safety to promote and assist with greater independence

24 hour, 7 days a week two-way verbal electronic communication with a central monitoring system to provide emergency access if and needed to members

Provide personal care assistance to achieve of activities of daily living; such as eating, bathing, dressing, personal hygiene, preparation of meals and light housekeeping tasks

Personal Support Services managed by individuals to direct their own care, by hiring, training and terminating their own care providers

Services provided to ensure consumers’ funds in the Plan of Care are managed and distributed as intended
