Ivy Health Case Management provides ongoing oversight, coordination, and monitoring of waiver-approved services to ensure members’ health, safety, welfare, and individualized needs are consistently met within the community setting. We manage the following services:

Assistance with activities of daily living like bathing, dressing, meal preparation, and light housekeeping. A consumer-directed option allows clients to hire and supervise their own support workers

Daytime, medically supervised care and group activities at an adult day health center

24-hour supervision, personal care, and meals in a state-licensed alternative residential setting, such as a personal care home

Temporary relief for primary family caregivers, available both in-home or overnight in an approved 24-hour facility

An in-home electronic device that allows participants to call for help at any time in an emergency

Nutritious meals delivered to the client’s home
